DOT Drug and Alcohol (SAP) Evaluations

Mary Lou Silvestros offers comprehensive DOT-SAP services in the Canonsburg, PA area. I schedule most DOT SAP evaluations within 24 hours. I will provide a written report to employer with education or treatment recommendations within 24-48 hours of the evaluation.

Due to changes in Department of Transportation regulations, evaluations can be conducted virtually via telehealth or face-to-face at our office. Typically, using telehealth option results in faster appointment times.
DOT SAP EVALUATIONS INCLUDE:
- Initial evaluation
- Recommendation for treatment and/or education (level determined by SAP)
- SAP monitoring of employee in education or treatment
- Written reports that meet DOT guidelines sent to the employer’s designated employee representative (DER) to outline recommendations and completion of recommendations
- Follow-up clinical evaluation to evaluate employee compliance
- Follow-up testing plan
- Aftercare recommendations (as necessary)
$450 fee for SAP Evaluations. Payment can be made via cash or credit card. No checks accepted.
Please note: If you are a CDL driver and have failed a DOT drug test, you must first register with the FMCSA Clearinghouse to request me as a SAP. This only applies to this industry and these new rules went into place January 2020. Your SAP process will not be able to be successfully completed unless you use this method.
